Important Information for Guaranteed Modified Whole Life Insurance policy kinds L770 (ID: L770ID; OK: L770OK; TN: L770TN; TX: L770TX):

We can issue only one insurance policy of this type on a guaranteed issue basis if between 45-85 (MO: 45-75; NJ: 45-79; OR: 45-80).

Full benefits are payable from the first day for accidental death. For non-accidental death during the first two years, benefits are limited to the premiums you’ve paid plus an extra 10%. After two years of coverage, the full benefit amount (less any outstanding loans against the insurance policy) is paid for death from any cause.

Important Information for Juvenile Life Insurance policy kind L726 (GA: L726L; ID: L726R; OK: L726K; TN: L726C; TX: Form L726Q):

Coverage amounts are subject to underwriting approval; issuance of this coverage may depend upon the answers to the health questions on the Application.

Full benefits are paid from the first day of coverage for any cause of death, except suicide during the first two years (in CO & ND: first year), which pays a return of premiums only, less any outstanding insurance policy loans.

Important information for Accidental Death and Dismemberment Rider LR111:

Death must result from injury and occur within 180 days from the date of injury.

We will not pay for death caused by:

Injury to which a contributing cause was a commission of or attempt to commit a felony or to which a contributing cause was being engaged in an illegal occupation; Intentional self-inflicted injury or suicide, while sane or insane; Injury due to flying, except as a fare-paying passenger on a regularly scheduled licensed airline; Injuries due to an act of war, declared or undeclared; Injury sustained while intoxicated or voluntarily under the influence of any drug, medication, or sedative unless prescribed by a physician; Injury sustained while participating in automobile or motorcycle racing activities or operating or riding in or on an off-road vehicle.
